To Submit a PO online follow these steps...
  1. Login and check your current price lists Price lists and marketing materials
  2. Fill out your PO form and save it as a PDF to your computer.
  3. Complete the PO submission form below and attach the saved PO from PDF by using the browse button. You may also fax your PO to us at (385) 695-5001.
  4. JTECH Medical will review your PO and you will receive an email response with your invoice shortly.
JTECH is happy to accept purchase orders from resellers via fax or email, that include the following information:
  1. Purchase order number
  2. Date
  3. Name of person authorizing the order
  4. Credit terms
  5. “Bill to” and “Ship to” addresses
  6. Product ordered
  7. Minimum order value $100
  8. Quantity ordered
  9. Total amount of purchase
  10. Shipping method: JTECH’s typical shipping method is UPS Ground.
  11. A $15 packaging fee will be as-sessed to all Freight Collect orders.
